Declaration of consent for processing of personal data

Pursuant to the Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46 / EC (Official Journal of the EU No. 119, page 1) – art. 12, art. 13. informs that:
I hereby express my consent to the processing of my personal data included in form by Zumbi Cycles Sp. z o.o., with its registered office in Myślenice (32- 400), Ul. Jana sobieskiego 44a  (the “Company”), for the purposes, execution of realization form, marketing and promotions regarding goods and services offered by the Company and its associated entities, including marketing purposes and execution of form requirements of the Company. I also give my consent to execution of form requirements.

The consent for the processing expressed above includes the following personal data: name and surname, e-mail address, phone number,  personal preferences and more information.

I hereby represent that I have been informed about my right to:
1. access and adjust my personal data,
2. lodge written, motivated request to cease processing my personal data,
3. lodge objection against processing my personal data,
4. as well as that providing my personal data is voluntary, but necessary to realization selling process

Pursuant to Article 10 paragraph 2 of the Act on Rendering Electronic Services dated 18 July 2002 (Journal of Laws of 2002, No. 144, item 1204 as amended) I hereby express my consent to receive from Zumbi Cycles Sp. z o.o., with its registered office in Myślenice (32- 400), Ul. Jana sobieskiego 44a commercial information in electronic way.
